The Community Development Template is a format
communities may use in recording the location of and use of
social, demographic, and health indicator data which will
be helpful to communities when applying for the Bureau of
Primary Health Care (BPHC) Section 330 Health Center Funding.
The Template contains a variety of data resources
that should assist a community in obtaining the types of information
needed to complete needs assessments and health related federal
grants proposals.
The underlying model for the Template is the
application for the Health Resources and Services Administration
(HRSA) - Bureau of Primary Health Care (BPHC), Public Health
Services, 330 Community Health Center New Access Point Application
(PIN 2003-01)
