Patients contact CHCA through a variety of methods, phone, fax, and e-mail. CHCA staff follows through with each contact to ensure that the patient receives the information and referral they are seeking. CHCA continually develops and refines tools that make the organization and dissemination of information about CHC locations and the services they provide as simple and user friendly as possible.

CHCA provides a toll-free call in number for patients to call and receive information and referrals to a CHC based upon their specific needs and location. All referrals are tracked on a monthly basis, and the information is used in the development of marketing and outreach efforts.
- Service Directory
The CHCA: CHC Service Directory is a master listing of services provided by each individual CHC. The Service Directory allows anyone to see, at a glance, exactly what services are provided and make provisions for other local providers to provide. CHCA: CHC Service Directory aids in the process of referring patients to the appropriate CHC based on their needs. The data is also used in writing grants, reports, marketing and educational materials, and white papers.
